Adult Disability Assistance
Personalized care
in your own home and community
Frontier has been providing services for individuals with developmental disabilities for over four decades.
We support independence and empower people to become self-advocates.
We believe people are healthier and happier living in their own homes and communities. Our team offers wrap-around services and advocates for the right of individuals with disabilities of all ages to make their own choices about where they live.
What it means to receive our wrap-around support
First, we find resources to cover your basic needs. Then we provide the pieces to help you pursue internal growth and independence and get out to connect with the community. Our goal is to empower people to live their best life by getting rid of barriers and offering access to all the services they need through all of life’s journeys.

Services
Case Management
Everyone we serve is assigned a community living coordinator who is the bridge of contact for families. They work closely with you to connect you with resources, including the basics like food, housing and transportation. They also help identify the Direct Support Professionals (DSP) who will assist with your individual needs and keep the care team connected throughout the duration of services with us.
In-Home Supported Living
Under 18 years old
We’ll come to your home to help your child develop everyday living skills to empower their independence. We help them move through routines such as dressing, meal preparation, daily chores, and even homework projects.

Adults 18+
We offer adults with disabilities a way to develop independent living skills. We’ll work with you in your own home and teach you how to prepare for your day and job, cook, grocery shop and all of the things it takes to be as independent as possible.
Assisted Living Homes for adults
There may come a time when someone can no longer safely live in their own home. Our group homes are a next step up in care. Unlike nursing homes, these are regular homes comfortable for a few people who are intentionally matched based on fit with others. They make the home their own.
Our specially trained staff are in the home 24/7 to provide support as needed, whether that’s with nursing oversight and getting to doctor’s appointments or community outings.
Respite Care for Caregivers
Primary caregivers work hard and deserve time for self care. That’s where we come in to help. We’ll take care of your loved ones while you take care of errands or use the time to rest.
Nursing Oversight
Our onsite nurse can help navigate questions about medical care, facilitate referrals to medical providers, serve as an advocate in medical appointments, and get needed equipment and supplies – all with a professional lens.
Chores for Seniors
Keeping up with household responsibilities can become overwhelming or too physically demanding. We’re happy to come by for a few hours a week to help with tasks around the house, including cleaning up, checking the mail and taking out the trash.
Community Engagement
Day Habilitation & Socialization
We love getting you out of the house and in the community for a variety of social activities based on goals in your plan and the things you enjoy most. Our staff helps to facilitate interaction and teaches social skills.
Community Outreach & Connection
Let’s get you connected with others and on your way to building skills that forge lasting friendships and invite community support. Our organized activities and outings are designed to foster fun and relationships. Whether we’re taking you on a trip or to local festivals and events, it’s about making connections and bringing joy into your life.
Adult Day Center for seniors 60+
Our staff are there to provide a safe and fun-filled day.
Join us in person at our Kenai Forget-Me-Not Center to meet people, explore new activities and rekindle old interests.
Seniors 60+ and anyone with Alzheimer’s, dementia or a related disorder are welcome to grab a prepared hot lunch and socialize with peers. Our active space has a free-flowing living room, games and craft tables where anyone can participate in a variety of multi-sensory activities that promote memory, create connection and bring joy. We’ll even provide transportation to get you there.

Activities are planned to encourage friendships, provide emotional support, promote physical activity and boost cognitive stimulation in a supervised environment. They are based on your choices, desires, and input. Each person's choice is respected and accommodated, even if they just want to relax and enjoy the view from our backyard.
